Job summary

Catholic Welfare Services, Singapore seeks an experienced social worker to lead comprehensive assessments and care planning for residents in elderly care settings. You will collaborate with families, multi-disciplinary teams, and external stakeholders to ensure holistic, resident-focused plans that uphold dignity and rights.

The role requires strong leadership, excellent communication, and a passion for improving residents’ quality of life within a regulated care environment.

Qualifications

• Bachelor’s or Master’s degree in Social Work from an accredited institution.
• Valid registration with the Singapore Social Workers Association (SASW).
• Minimum of 7 years of relevant experience in social work, particularly in geriatrics/elderly care.
• Strong communication and interpersonal skills with ability to interact with elderly residents, families, and a multidisciplinary team.

Responsibilities

• Conduct bio-psychosocial-spiritual assessments and coordinate inter-agency conferences for complex cases.
• Develop and implement resident care plans and intervention strategies across the resident’s trajectory.
• Lead multidisciplinary team or inter-agency case conferences and advocate for residents’ dignity, confidentiality and rights.
