Job summary

DEMENTIA SINGAPORE LTD. in Singapore seeks a dedicated Care Coordinator to support people with dementia and their carers. You will assess cases, formulate individualized care plans, provide psychoeducation and supportive counselling, and arrange referrals as needed.

You will collaborate with a multidisciplinary team, document progress, and contribute to programme evaluation. Occasional Saturdays/evenings may be required to meet client needs and events.

Qualifications

• Preferably with degree in relevant allied health profession such as social work, psychology, occupation therapy.
• Good oral, written and interpersonal communications skills.
• Ability to speak a second language and/or dialect(s) will be an added advantage.
• Ability to facilitate group sessions will be an added advantage.
• Able to work on occasional Saturdays and weekday evenings.
• Passionate about dementia care and working with people.
• Prior working experience in dementia care will be an added advantage.

Responsibilities

• Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
• Provide intervention such as psychoeducation, supportive counselling and service referral.
• Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
• Provide case recordings and relevant documentations.
• Participate in team meetings and case conferences as required.
• Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
• Plan and facilitate Caregiver Support Group sessions.
• Support in outreach events and talks.
• Any ad-hoc tasks as and when required.
