Qualifikationen

• Degree in Social Work (SW) and registration with SASW or equivalent.
• Good written and spoken English.
• Experience in nursing home settings is an advantage.

Aufgaben

• Assess referrals and coordinate placement applications.
• Conduct means testing and social/financial assessments.
• Provide financial counselling and assist with financial applications.
• Collaborate with multidisciplinary team for holistic care planning.
• Liaise with government agencies and healthcare organisations.
• Maintain case records and ensure confidentiality under PDPA.
• Prepare reports and documentary submissions for accountability.
