Job summary

Qualifications

• Minimum 2 years of experience in community engagement and/or case management.
• Good oral, written and interpersonal communication skills.
• Ability to conduct Advance Care Planning conversations or prior experience with persons with dementia or carers is an added advantage.
• Willing to work on Saturdays and occasional weekday evenings.
• Ability to facilitate group sessions will be an added advantage.
• Self-motivated with a high level of initiative.
• Resourceful and adaptable.
• Strong attention to detail.
• Good organizing and execution skills.

Responsibilities

• Conduct case assessment and formulate individualized care plan for persons with dementia and their carers.
• Plan and facilitate group sessions/activities for carers and/or persons with dementia.
• Provide intervention such as psychoeducation, supportive counselling and service linkages.
• Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
• Provide timely and accurate case recordings and relevant documentations.
• Participate in team meetings and case conferences as required.
• Assist promptly in the preparation of data and statistics for programme evaluation and reporting purposes.
• Explore new avenues of collaboration with community partners and stakeholders.
• Support in outreach events and talks.
• Any ad-hoc tasks as and when required.
