Job summary

You will collaborate with community partners, support operations planning, monitor KPIs, pilot new programmes, and help enhance patient experience by managing feedback and complaints.

Qualifications

• Bachelor's degree required.
• Experience in healthcare, operations or customer service environment.
• Minimum 2 years of experience.
• Strong writing and communication skills.
• Good interpersonal skills and sense of responsibility.
• Ability to manage complaints and feedback effectively.

Responsibilities

• Lead community care team operations in an assigned area to improve residents' health and anchor care in the community.
• Support community care team operations to help residents lead a healthy lifestyle through community operations (e.g., Transition care programme, Community Health Posts).
• Engage community partners to coordinate care and operations in the area.
• Support operations planning and monitor workload and KPIs.
• Pilot new programmes and processes; evaluate pilot success for future action.
• Support overall Patient Experience efforts, manage feedback from all sources, and address residents' concerns.
• Any other duties as required.
