Job summary

NUHS Population Health & Community Care (PHCC) invites applications for a Regional Health Manager to lead community care operations in western Singapore. You will collaborate with NUHS hospitals and polyclinics, GP partners and community providers to anchor care in the community and improve residents’ health experiences.

The role emphasizes programme development, coordinating community operations, evaluating services, and driving patient experience improvements while strengthening partnerships

Qualifications

• Bachelor's degree in a related field or equivalent experience.
• Minimum 2 years of working experience in healthcare, operations or customer service.
• Experience in healthcare operations and customer service is advantageous.
• Strong written, verbal, and interpersonal communication skills.
• Ability to manage complaints and feedback with a sense of responsibility.

Responsibilities

• Support the operations of Community Care Team in assigned area to improve health and anchor care in the community.
• Engage community partners to collaborate in taking care of residents' health and coordinate community operations.
• Support operations planning in the assigned area, including data analysis and KPI monitoring.
• Assist in improving community operations to enhance care and residents' experience.
• Support overall Patient Experience efforts and address residents' concerns with stakeholders.
• Other duties as assigned.
