Job summary

SPD LTD. in Singapore is seeking a Case Management Officer to support interventions, community development, and professional practice in social services.

You will coordinate casework, group activities for clients and caregivers, and assist with administrative tasks to ensure smooth service delivery. You will perform needs assessments, coordinate referrals, provide psycho-emotional support, and work with a multi-disciplinary team to plan and deliver programmes.

Qualifications

• Possess a recognized Degree or Diploma in Social Work, Psychology, Nursing, Health Sciences, or a related discipline.
• Experience in social service sector or related discipline preferred.
• Analytical, flexible, resilient and ability to see different perspectives and draw connectivity and suitably make recommendations.
• Committed, responsible, able to work independently and be a contributing team player.
• People-oriented and have good interpersonal skills.
• Effective report writing skills.
• Able to work in the evenings and weekends, where needed.

Responsibilities

• Case work management: conduct intake assessments and provide social reports with recommendations.
• Coordinate service referrals and access to community resources.
• Provide psycho-emotional support for clients and caregivers.
• Lead community outreach and caregiver group work and training.
• Manage projects and participate in professional development and supervision.
• Provide data for progress reports and participate in cross-divisional projects.
• Support evening/weekend outreach as needed.
