Job summary

Qualifications

• Degree in Social Work from an accredited institution.
• Registered Social Worker (RSW).
• Excellent communication and interpersonal skills with demonstrated ability to build relationships.
• 7+ years of experience in social work, preferably in disability sector.
• Minimum 3 years of supervision experience.

Responsibilities

• Case work management across varied risk levels.
• Conduct needs and intake assessments and prepare social reports with recommendations.
• Coordinate service referrals and access to community resources.
• Provide psycho-emotional support to clients and caregivers.
• Build partnerships with agencies to tap community resources and support transitions into community living.
• Lead and participate in multidisciplinary team discussions and reporting.
