Job summary

SPD Rehabilitation Center in Singapore seeks a Social Worker/Case Management Officer to deliver evidence-based interventions, conduct intake assessments, and coordinate referrals with a multi-disciplinary team. Candidates should hold a degree in Social Work or Counselling, with 2–3 years of relevant experience and RS W accreditation.

Qualifications

• Degree in Social Work, Counselling or related field.
• Registered Social Worker (RSW) accreditation preferred.
• 2–3 years of social work or counselling experience preferred.
• Strong analytical, communication and report writing skills.
• Able to work in evenings and weekends when needed.

Responsibilities

• Conduct intake assessments and social reports with recommendations for independent living.
• Manage case work for varying risk levels and coordinate referrals.
• Provide psycho-emotional support to clients and caregivers.
• Build partnerships with agencies to access community resources.
