Job summary

SPD Singapore is seeking a Social Worker/Case Management Officer to deliver evidence-based interventions within a rehabilitation setting, coordinating case work, community outreach, caregiver support, and project initiatives.

You will work with a multidisciplinary team to assess needs, plan interventions, document outcomes, and support clients and families toward independent living and successful reintegration into the community.

Qualifications

• Possess Degree in Social Work, Counselling or Degree with a Graduate Diploma in Social Work from an accredited institution.
• Accredited as a Registered Social Worker (RSW) if relevant.

Responsibilities

• Case Work Management: conduct needs and intake assessments and provide social reports with recommendations.
• Coordinating service referrals and access to community resources.
• Provide psycho-emotional support for clients and caregivers.
• Community Work and Outreach: identify changing needs and initiate outreach activities.
• Caregiver Support, Groupwork and Training: lead caregiver support, conduct psycho-educational training and organise support groups.
• Project Management: lead and drive community projects and social work initiatives.
• Professional Development / Supervision: participate in developmental programs and supervision.
