Job summary

MINDSG LTD is seeking a Care Coordinator to support the national pilot of the Caregiver Support Framework and Person and Family-centred Planning (PFP) for PWIDs and their caregivers. The role involves engaging, assessing, and coordinating services across health, social services, and community resources to deliver holistic support.

You will act as the primary contact for caregivers and PWIDs, ensuring smooth delivery of the programme and contributing to capacity-building and cross-sector

Qualifications

• Degree in Social Work, Psychology, Counselling, or related social service fields.
• Experience in social services, case management, disability services or person-centred planning preferred.

Responsibilities

• Coordinate Programme Coordination & Support for Future Care Planning activities, including assessing PWIDs and caregivers and co-developing tailored care plans.
• Provide ongoing case management through follow-ups, screenings, and tracking progress of care plans.
• Maintain confidential records for internal reporting and for submission to SG Enable (SGE) for evaluation.
• Build relationships with stakeholders, managing communications and contributing to programme evaluation and workflow improvements.
