Job summary

MINDS is seeking a dedicated Care Coordinator to join the Community-Based Support Services team. You will report to the Head of Caregivers Support Services and work with PWIDs and their caregivers to conduct structured screening, tailor care plans, and coordinate services across sectors.

You will act as the primary contact for caregivers and PWIDs, raise awareness of the Caregiver Support Framework, and ensure smooth delivery of the programme while maintaining confidential records and

Qualifications

• Degree in Social Work, Psychology, Counselling, or related social service fields from recognised tertiary institutions
• Experience in social services, case management, disability services or person-centred planning preferred

Responsibilities

• Coordinate Caregiver Support Services and engage PWIDs and caregivers
• Administer screening tools and develop tailored care plans
• Facilitate access to services across healthcare, social services, financial schemes, and community programmes
• Maintain confidential records and report to SG Enable (SGE) for reporting and evaluation
• Build and maintain relationships with stakeholders and participate in programme evaluation
