Job summary

People Profilers Pte Ltd is hiring a Care Navigator to support the Care Guides Programme in Singapore. The role involves outreach, screening caregivers, facilitating matching, onboarding, and ongoing coordination with caregivers and Care Guides to ensure smooth operations.

You will manage digital touchpoints, handle scheduling and record-keeping, assist with training sessions, and work closely with the Senior Medical Social Worker. A Diploma in Social Work or related field is required.

Qualifications

• Diploma in Social Work, Counselling, Gerontology or related field.
• 1–3 years in social service, community work, or healthcare.
• Excellent interpersonal and communication skills for engaging caregivers.
• Organised and detail-oriented with strong administrative ability.
• Interest in caregiver support and community-based services.

Responsibilities

• Support outreach, engagement, and publicity for Care Guides.
• Conduct initial contact, screening and needs assessment of caregivers.
• Coordinate matching and onboarding between Care Guides and caregivers.
• Provide ongoing coordination and follow-ups with caregivers and Care Guides.
• Assist in managing programme touchpoints on digital platforms and inquiries.
• Provide administrative support including scheduling, data entry, and records.
• Support data tracking, monitoring and reporting of activities.
• Organise training sessions and engagement activities for Care Guides and caregivers.
• Work with Senior Medical Social Worker to ensure coordinated care.
• Assist in caregiver follow-ups and home visits under supervision.
• Participate in supervision, training and team meetings.
• Perform other duties as assigned.
