Clinical Care
- Monitor client’s vital signs and weight by measuring, recording and reporting the data gathered
- Report and assist with intervention for any deviation from baseline
- Develop and review ICP for clients every 6 month
Monitor Cognitive Level in Clients
- Monitor cognitive level in clients and participate in developing appropriate intervention in consultation with qualified healthcare professionals
- Observe for any change in behavior which may be possible signs of Dementia
- Initiate simple strategies to de‑escalate onset of episode
Medical Assistance
- Communicate with clients in relation to remind them to take their pre‑packed oral medication and or applying prescribed topical medication in accordance
- To observe the 4 rights (client, quantity, route, time)
- Observe clients for side effects
Provide Personal Hygiene Support
- Provide personal hygiene and grooming support, including skin care, for clients with mobility and ambulation problems
Perform Oral Feeding for Client
- Support client with or without swallowing difficulties in oral feeding
- Encourage client participation to maximize independence
Assist on Tube Feeding
- Support client by performing a safe tube feeding process
Monitor Adherence to Diet Plan
- Observe, identify, record and report client’s nutritional intake and adherence to diet plan
Activities of Daily Living (ADLs)
- Provide care and assistance in ADLs accordance with their care plans
- Recognize boundaries and consult relevant personnel when needed
- Perform client’s transfers safely and provide feedback on transfer status
- Maneuver client’s wheelchair with good safety awarenes
- Identify and understand needs and concerns of clients with different physical abilities
Conduct Structured Activities
- Conduct and supervise one‑to‑one and group structured activities
- Conduct physical activities suitable with the right challenge activities for clients with different physical abilities
- Conduct activities suitable for persons with Dementia
- Accurate documentation of client’s engagement in Salesforce
Engage Clients with Person Centered Care (PCC) Approach
- Able to use PCC as a guiding principle when engaging clients
Emergency Response and Crisis Management
- Able to support responsibilities as prescribed in Emergency response and crisis management plans and work with external and internal stakeholders to conduct periodic reviews and ensure emergency preparedness
Stakeholder Management
- Able to Support and manage constructive and positive relationships with stakeholders (internal and external)
- Provide support to volunteers, interns and students attached to the Centre
Administrative Management
- Ensure that all records of clients in Salesforce are updated
- Understand and comply with SOPs and MOH service requirement
- Ensure that Centre is ready for audit
Job Requirements
- At least a GCE “N” level / HMI / WSQ related study in Healthcare
- At least 2-3 years of relevant experience in Eldercare
- IT savvy
- Must enjoy working with elderly with physical disabilities and/or dementia in Centre‑based setting
- Able to multi‑task and work in a physical environment that is dynamic
- Able to work with a team with people of all levels and adaptable to the potential need of change of job tasks
