Job Description

As a Home Health Nurse at Freelan, you will be responsible for providing comprehensive and compassionate nursing care to patients in their homes. You will work closely with a multidisciplinary team to assess, plan, implement, and evaluate patient care plans. Your primary goal will be to promote the health and well-being of our patients and support them in achieving their optimal level of independence.

Responsibilities

• Conduct initial and ongoing assessments of patients\' physical, emotional, and psychosocial needs
• Develop and implement individualized care plans based on patients\' needs and goals
• Administer medications and treatments as prescribed by the physician
• Monitor patients\' progress and make necessary adjustments to care plans
• Educate patients and their families on disease management, medication management, and self-care techniques
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive care
• Provide emotional support and counseling to patients and their families
• Maintain accurate and up-to-date patient records
• Adhere to all professional, ethical, and legal standards of nursing practice
• Participate in ongoing training and development opportunities to enhance skills and knowledge
