Care Center Clinic is seeking an Encoder Associate to oversee PhilHealth YAKAP data entry and record keeping. The role emphasizes accurate encoding of patient information, health assessments, and documentation within the YAKAP program.

The incumbent will coordinate with clinicians and admin staff, ensure data integrity, and maintain confidentiality of patient records. This position requires meticulous attention to detail and adherence to policies. Training provided.

Qualifications

• Data entry accuracy and efficiency in health records.
• Knowledge of health information privacy and data security practices.
• Experience with medical records systems is a plus.

Responsibilities

• Encode and maintain patient information in the PhilHealth YAKAP system.
• Assist in processing and verifying patient registration and membership data.
• Encode medical consultations, health assessments, and other required data.
• Ensure completeness and accuracy of encoded information per guidelines.
• Review and validate documents and records for accuracy.
• Coordinate with providers and staff on encoding requirements.
• Maintain confidentiality and security of patient records.
• Monitor and update records for timely submissions.
• Prepare reports and documentation for PhilHealth YAKAP services.
• Perform other administrative and encoding tasks as assigned.
