Responsibilities

• Process patient applications for DHC waiver programs accurately and on time.
• Draft and maintain Individual Support Plans (ISPs) with clear patient needs and services.
• Follow up with families to obtain missing information or documents.
• Manage multiple applications and ISPs with 100% accuracy and attention to detail.
