Qualifications

• Bachelor's degree in healthcare, nursing, or related clinical field.
• 2+ years in customer service or healthcare, with care coordination experience.
• Solid understanding of the US healthcare system and its processes.
• Excellent written and verbal English communication skills.
• Strong attention to detail, organization, and ability to multitask in a fast-paced setting.
• Familiarity with CRM and EMR systems; proficiency with common office tools.

Responsibilities

• Guide patients through onboarding and coordinate with clinical staff.
• Manage inquiries via phone, email, and other channels with timely responses.
• Process referrals and support conversion timelines.
• Enter clinical data, RPM/RTM data streams, and care notes into EMR.
• Schedule and coordinate patient appointments, preserving care continuity.
• Handle inbound/outbound medical records and maintain organized documentation.
• Attend clinical meetings and capture actionable items.
