Ascension Business Solutions LLC is seeking a Chart Scrub Virtual Assistant to support care management by reviewing patient records, identifying overdue follow-ups, coordinating scheduling, and routing clinical concerns to licensed professionals. The role ensures accurate patient charts and continuity of care by addressing missed appointments, incomplete referrals, and documentation changes.

The VA does not diagnose or interpret clinical results and must escalate all clinical matters to licensed

Qualifications

At least one year of experience in healthcare administration, scheduling, or care coordination.
Experience reviewing electronic health records and patient charts.
Strong written and verbal English communication skills.

Responsibilities

Review patient charts using approved workflow and verify active status, provider, and contact info.
Classify charts by follow-up type and route to the correct queue with clear reasons.
Contact patients to verify details and schedule routine or priority follow-ups.
Escalate urgent or unclear cases to licensed clinical staff per procedures.
Document chart reviews, outreach attempts, and outcomes in the system.
