Job summary

Core-VA Solutions is seeking a Case Management Specialist to help individuals receiving long-term care services. You will coordinate care, manage documentation, and serve as a trusted contact for clients, families, and providers in English and Spanish.

This 100% remote role offers structured training, PTO, paid U.S. holidays, and a collaborative, people-focused team dedicated to person-centered care.

Qualifications

• 2+ years in case management or care coordination.
• Fluency in Spanish and English (written and verbal).
• Experience with U.S.-based clients is a plus.
• Scheduling, documentation, and reporting in healthcare or social services.
• Proficiency with case management software, EHRs, or MS Office.
• Discretion handling sensitive information; detail-oriented.
• Able to work independently and in a team in a fast-paced setting.

Responsibilities

• Maintain accurate case files, service plans, and records with confidentiality.
• Schedule and coordinate appointments, referrals, and reviews.
• Liaise between clients, families, providers, and community orgs in English and Spanish.
• Develop, monitor, and update individualized service plans and deadlines.
• Ensure services meet state and agency regulations.
• Prepare reports on client progress and outcomes.
• Provide interpretation/translation support for Spanish-speaking clients when needed.
• Offer feedback to improve workflows and client satisfaction.
