Job summary

Core-VA Solutions is seeking a Case Management Specialist to support individuals receiving long-term care services from home. You will coordinate care, manage documentation, and act as a trusted contact for clients, families, and providers in English and Spanish. This role offers remote work with meaningful impact.

You will maintain confidential case files, schedule appointments, and develop service plans while ensuring compliance with state regulations and agency standards.

Qualifications

• 2+ years of experience in case management or care coordination.
• Fluency in English and Spanish (written and verbal).
• Experience with U.S.-based clients.
• Experience with scheduling, documentation, and reporting in healthcare or social services.
• Proficiency in case management software and EHR systems.

Responsibilities

• Maintain accurate case files and records with confidentiality.
• Schedule and coordinate appointments, referrals, and reviews.
• Liaise among clients, families, providers, and community orgs in English/Spanish.
• Develop and update individualized service plans and track deadlines.
• Ensure services and documentation meet regulations.
• Monitor compliance with state and agency standards.
• Support person-centered care with providers and coordinators.
• Prepare reports on client progress and outcomes.
• Resolve care concerns and remove barriers to accessing services.
• Provide interpretation/translation for Spanish-speaking clients (nice to have).
• Provide feedback to improve workflows and client satisfaction.
