Qualifications

• 2+ years of experience in case management, care coordination, or similar role in healthcare or human services.
• Fluency in Spanish and English, written and verbal (nice to have).
• Experience with scheduling, documentation, and reporting in healthcare or social services.

Responsibilities

• Maintain accurate case files, service plans, and records with confidentiality and compliance.
• Schedule and coordinate appointments, referrals, service authorizations, annual reviews, and follow-ups.
• Liaise between clients, families, providers, and community organizations in English and Spanish (nice to have).
• Develop, monitor, and update individualized service plans with deadlines and progress tracking.
• Ensure services and documentation meet state and agency regulations.
• Monitor compliance with state and agency regulations for service delivery and documentation.
