Qualifications

• Degree in social work, nursing or healthcare is advantageous.
• Strong interpersonal communication and stakeholder management are essential.
• Reporting to the Head of Regional Services in a fast-paced environment.
• Location at Toa Payoh, Singapore.
• 2-year contract commitment required.
• Salary up to 5900 SGD.

Responsibilities

• Implement Care Coordination programme activities and workplans.
• Pilot joint referral workflows and care coordination initiatives.
• Conduct interRAI care assessments and care planning.
• Serve as primary liaison with partner organisations in sub-regions.
• Maintain understanding of services and align offerings across sub-regions.
• Represent ICCP in sub-regions and foster partnerships.
• Support community outreach and stakeholder collaboration.
• Organise ICCP Steering Committee and operational meetings.
• Act as liaison between internal teams and external partners.
• Monitor service delivery and progress against targets.
• Identify service gaps and support improvement initiatives.
• Assist in preparing reports, dashboards and presentations.
• Maintain oversight of sub-regional involvement with milestones.
