My Guardian in the Philippines seeks a Clinical Care Coordinator - Registered Nurse to lead nursing care coordination, develop and maintain client care plans, and provide specialist nursing guidance to Home Care staff. You will oversee wound care, therapy administration, and liaise with hospitals and doctors as part of a multi-disciplinary care team.

Responsibilities

• Attend home assessments and create, implement and maintain client care plans.
• Provide nursing guidance to Case Managers and Home Care Workers.
• Maintain up-to-date client plans and records.
• Wound care and dressing changes.
• Coordinate with hospitals and allied health professionals.
• Liaise with and organise allied health as required by clients.
• Report changes in a client’s health or behaviour.
• Administer treatments and medications with knowledge of adverse effects.
• Mentor peers to maintain professional nursing standards.
