Job summary

CT Prime-Solutions Virtual Assistance Services is seeking an Intake Coordinator to manage the intake and enrollment process for patients, caregivers, physicians, and state caseworkers. Responsibilities

• Manage the intake and enrollment process for new patients.
• Coordinate the collection and completion of enrollment documents.
• Obtain physician-signed medical necessity forms and other required clinical paperwork.
• Communicate with state social workers and caseworkers to secure and maintain service authorizations.
• Assist patients and caregivers with required documentation and enrollment requirements.
• Train caregivers on clock-in/clock-out procedures and EVV requirements.
• Maintain accurate records and documentation.
• Follow up on pending paperwork to prevent service interruptions.
• Serve as the primary point of contact for patients, caregivers, and referral sources.
• Ensure all enrollment and authorization requirements are completed accurately and on time.
