ReadyTeam in the Philippines seeks an Intake Coordinator for our Virtual Home Health / Hospice team. You will support referrals and admissions with accuracy, organization, and efficiency from start to patient start.

Responsibilities include reviewing referrals, verifying documents, coordinating with sources, entering patient details, and updating clinical and administrative staff to improve turnaround times and patient experience. This is a training-friendly, month-long onboarding.

Qualifications

• From the Philippines and doesn't have any dual citizenship.
• Excellent English communication skills.
• Home health or hospice experience.
• Strong organizational and time-management skills.
• Detail-oriented with high accuracy.
• Ability to multitask and prioritize work.
• EHR/EMR experience.
• Strong computer skills and ability to learn new software.
• Willing to be trained for a month.

Responsibilities

• Receive and review referrals for accuracy and completeness.
• Verify intake documents and request missing records.
• Coordinate with referral sources and schedule start-of-care visits.
• Enter patient demographics and insurance details.
• Communicate updates to clinical and administrative staff.
