Job summary

E-Teleconnect in Paranaque City, Philippines, seeks an Afterhours Intake professional to manage referrals, coordinate onboarding, and ensure timely transition of care from hospital to home health services. This non-bedside, operations-based role emphasizes documentation, intake coordination, and initial patient contact.

You will review referrals, organize documentation, perform initial clinical review for home health eligibility, and support handoffs to Case Management and Scheduling teams,

Qualifications

• Minimum 2 years in case management, intake, or care coordination.
• Experience with EMR/EHR systems (HHMD, Axxess, Kinnser/WellSky).
• Strong documentation and data-entry skills.
• Ability to work independently in a fast-paced operations environment.
• Excellent verbal and written communication.

Responsibilities

• Receive and manage referrals via fax, email, and chat platforms.
• Review referral packets for completeness including H&P, orders, discharge notes, authorizations, and supporting clinical documents.
• Organize and upload all documentation into the EMR system.
• Ensure documentation is complete and ready for Case Management endorsement.
• Conduct initial clinical review to determine eligibility for home health services.
• Identify missing documentation or inconsistencies and escalate accordingly.
• Coordinate handoff to Case Management and Scheduling teams.
• Accurately input patient demographics and clinical data into EMR/EHR systems.
• Track referral status and pending documentation.
