Job summary

MicroSourcing is seeking a Home Health Intake Specialist in Pasay City to coordinate referral intake, engage with the care team, patients, and families. The role may involve in-home visits for clinical duties, aligned with nursing qualifications.

Qualified LVN/LPN candidates with active licensure and a current driver’s license are preferred; CPR is mandatory if in-home visits are required.

Qualifications

• Education: Vocational/practical nursing preferred; non-clinical must have high school diploma or equivalent.
• Licensure: LVN/LPN current state license; multi-state NLC preferred.
• Current driver’s license; CPR card if in-home visits are required.

Responsibilities

• Coordinate referral intake with the interdisciplinary team, patients, their families, and referral sources.

Your Role

• Facilitate the referral intake process, coordinating care with the interdisciplinary team, patients, their families, and referral sources.
• Perform in‑home visits when required (clinical roles).

Qualifications

• Education: Graduated from an accredited vocational/practical nursing program is preferred; if non‑clinical, a high school diploma or equivalent is required.
• Licensure: Clinical candidates must hold a current state license as a Licensed Vocational Nurse or Licensed Practical Nurse. Multi‑state license issued by a state member of the National Nurse Licensure Compact (NLC) is preferred.
• Current driver’s license.
• If in‑home visits are required, a current CPR card is mandatory.

Preferred Experience & Skills

• One year of experience as a Licensed Vocational/Practical Nurse, with home‑care experience preferred.
• Proficient nursing skills aligned with generally accepted standards of practice.
• Good interpersonal skills, with an ability to establish and maintain effective relationships with staff, patients, families, and the public.
