Job summary

E-Teleconnect is seeking a Case Management Coordinator in Parañaque City for on-site support. You will coordinate patient cases, manage referrals and documentation, and liaise with healthcare providers and insurance representatives to ensure timely progression through the care continuum.

You will work with RN Case Managers, physicians, facilities, and internal teams to ensure efficient patient care operations and accurate records in EMR/EHR systems.

Qualifications

• Minimum 2 years combined experience in case management, care coordination or related fields.
• Familiarity with US healthcare workflows and BPO environments is a plus.
• Experience with EMR/EHR documentation and coordination activities.

Responsibilities

• Coordinate assigned patient cases throughout the care continuum and track progression.
• Review and ensure completion of required documentation and referrals.
• Support RN Case Managers by handling administrative care coordination tasks.
• Escalate clinical questions to the RN Case Manager when needed.
