Job summary

E-Teleconnect, Inc. is seeking an Afterhours Intake Coordinator to manage referrals, review documentation, and onboard patients for Supportive Home Health. This non-bedside, operations-focused role emphasizes accurate data entry, EMR management, and clear communication with care teams.

You will perform initial clinical triage, identify missing documents, and coordinate handoffs to Case Management, Scheduling, and Quality Assurance to ensure timely admission and integrated care delivery.

Qualifications

• Minimum 2 years in case management, intake coordination, or related healthcare ops.
• Experience using EMR/EHR systems such as HHMD, Axxess, or Kinnser/WellSky.
• Strong data entry, documentation review, and attention to detail.
• Ability to work independently in a fast-paced operations setting.

Responsibilities

• Receive and manage referrals via fax, email, and chat.
• Review packets for completeness and organize in the EMR.
• Conduct initial clinical review to determine eligibility for home health.
• Coordinate with Case Management and Scheduling for admission readiness.
