Job summary

HealthForce NZ is seeking a Senior Practice Nurse to lead and deliver high-quality, patient-centred care in a community-focused primary care setting in New Zealand. You will work within a collaborative multidisciplinary team, guiding advanced nursing practice and contributing to chronic disease management, immunisation programs, and health promotion for diverse whānau.

Qualifications

  • Current APC from New Zealand Nursing Council.
  • +5 years’ experience in General Practice is preferred.
  • Vaccinator certification is required.
  • Cervical Smear Certification is preferred.
  • Strong autonomy with excellent assessment and time management.
  • Experience with diverse patient populations.

Responsibilities

  • Lead chronic disease management and preventative health initiatives.
  • Provide immunisation services and care planning.
  • Contribute to quality improvement and nursing service development.
  • Collaborate with multidisciplinary team to deliver patient-centred care.
  • Support equity-focused approaches for Māori and Pacific communities.

Position overview

Bring your extensive Practice Nursing experience, clinical expertise, and passion for equity-focused healthcare to a progressive and community-centred medical practice. This is an exciting opportunity to play a key leadership role in delivering high-quality, patient-centred care to a large and diverse population, with a strong focus on supporting Māori and Pacific communities to achieve improved health and wellbeing outcomes.
As a Senior Practice Nurse, you will work at the forefront of primary healthcare, providing advanced nursing knowledge and clinical guidance within a collaborative multidisciplinary team. You will build trusted relationships with patients and their whānau, empowering them to actively manage their health while navigating complex healthcare needs with compassion, cultural responsiveness, and manaakitanga.
This is a varied and fast-paced role that offers the opportunity to make a meaningful difference every day. Working alongside a multicultural team, you will contribute to chronic disease management, preventative health initiatives, health promotion, immunisation programmes, and care planning, while supporting the ongoing development of nursing services and quality improvement initiatives across the practice.
This is a full-time position, offering 36-40 hours per week, Monday to Friday, with occasional Saturday morning clinics.

Essential skills and experience
  • Current Annual Practicing Certificate from New Zealand Nursing Council
  • Ideally +5 years’ experience working in General Practice
  • Current Authorised Vaccinators Certificate
  • Cervical Smear Certification (preferred)
  • A high degree of autonomy, with great assessment and time management skills
  • Experience working with patients from diverse cultural backgrounds
