Job summary

Pathways Health Ltd in Wellington is seeking a Health Coach/Support Worker to join our mobile service. This permanent, 32-hours-per-week role runs Monday to Thursday, 8:30am–5:00pm, based in Central Wellington.

You will report to the Team Coach and work with a supportive team to provide medication support, assist with meals and daily living tasks, and help people on their mental health journey regain independence. While mental health experience is preferred, lived experience is valued.

Qualifications

• Current NZ driver’s licence.
• First aid certificate or willingness to obtain one.
• Knowledge of Te Tiriti o Waitangi and its application.
• Level 4 NZ Certificate in Health and Wellbeing (NZCHW) or equivalent, or actively studying toward it.

Responsibilities

• Provide medication support and recovery-focused care.
• Assist with meal preparation and household tasks.
• Empower independent daily living skills and goal achievement.
• Foster positive community connections and strong foundations.
• Work alongside Registered Health Professionals with a common purpose.
• Manage your caseload with the support of a collaborative team.

Education

NZCHW Level 4 or equivalent (or studying toward this)

We’re looking for passionate individuals who thrive on autonomy and value independence.

You will also have:

• A current full NZ driver’s licence.
• A current first aid certificate or be willing to obtain one.
• Knowledge of, and demonstrable commitment to, the principles of the Te Tiriti o Waitangi
• The Level 4 New Zealand Certificate in Health and Wellbeing (NZCHW), or a recognised equivalent — or be currently studying toward this qualification or equivalent.
