Health New Zealand | Te Whatu Ora is seeking a Coordinator - Coasters Home & Community Support Services on a full-time basis, 40 hours per week, to coordinate care for clients living well, aging well and with dignity in their communities.
Working with service referrers, coasters nurses, support workers and leads to arrange rosters, provide high-quality customer service, uphold cultural safety and confidentiality, and support ongoing training and development.
The Coordinator - Coasters Home & Community SupportServices, is responsible for coordinating care to support consumers to livewell, age well and have a respectful end of life in their owncommunities.As a Coordinator, you will work closely with our ServiceReferrers and Coasters Nurses on a daily basis to arrange service requests forclients. You will also liaise with Support Workers and our ACNM or AH leadregarding rostering, including covering annual leave, sickness, and training.Providing high-quality customer service to clients, their whanau, and familiesis also a key part of the role.
