The Advancing Wellness at Home Initiative (AWHI) is seeking an enthusiastic and motivated Team Coordinator to support our growing service and play a key role in its ongoing development and success.
This position combines clinical coordination, leadership, and service development responsibilities. A key focus of the role is the coordination of daily clinical operations through an in-reach screening model. Working closely with inpatient teams, the Team Coordinator will identify and assess patients who are suitable for AWHI services, facilitate timely discharge planning, and oversee the allocation and prioritisation of clinical workloads across the team.
In addition to coordinating clinical operations, the Team Coordinator will contribute to workforce planning and rostering, staff recruitment, performance and development activities, and continuous quality improvement initiatives. The successful candidate will also provide clinical support and guidance to team members as required, ensuring the service continues to deliver safe, effective, and patient-centred care.
The Coordinator role is based at Hutt Hospital and provides support across all AWHI streams. AWHI aims to facilitate timely hospital discharges and deliver nursing and allied health services, including re-enablement, to patients in their own homes.
The team primarily works with older adults who have been admitted to hospital following an acute medical event, stroke, or injury, supporting them to achieve their rehabilitation goals.
In this role, you will contribute to leading a collaborative, supportive, and motivated multidisciplinary team comprising Physiotherapists, Occupational Therapists, Speech‑Language Therapists, Social Workers, Nurses, and Assistants.
We offer ongoing professional development opportunities, including discipline‑specific training and interprofessional practice support. You will also have access to professional guidance from relevant professional leaders, as well as clinical supervision. Cultural professional supervision is available through Māori and Pacific Health teams.
We are looking for an Allied Health Clinician or Registered Nurse who has current registration/APC and has recent clinical experience working with older adults or in rehabilitation/community settings.
