Job summary

Careway in Auckland’s Mt Eden is seeking a Care Coordinator to join the Referrals and Triage team. You will screen referrals, connect with patients, gather information for triage, and coordinate next steps across our provider network.

You will contribute to timeliness, continuity, and clear pathways from first contact, while maintaining patient privacy and cultural safety in every interaction. This role supports flexible working hours and is based in Mt Eden, Auckland.

Qualifications

• Experience in healthcare coordination, referrals and rehabilitation.
• Familiarity with ACC services and medical administration.
• Strong written and oral communication with patients and providers.

Responsibilities

• Receive and review referrals and supporting information for accuracy.
• Check claim status, ICPMSK eligibility and pathway requirements.
• Contact patients to explain the service, confirm details and obtain consent.
• Gather clinical, functional and contextual information for triage.
• Coordinate pathway entry and onward navigation to Careway services.
• Prepare clear documentation within agreed timeframes.
• Collaborate with the interdisciplinary team and escalate as needed.
• Contribute to service quality and referral flow.
• Maintain patient privacy and cultural safety in all touchpoints.
