Job summary

Total Care Health Service in Auckland is seeking a Clinical Coordination Lead to provide clinical expertise and client-centred coordination for community nursing across Aotearoa. You will support safe entry to service, clinical triage, and coordination through to discharge with cross-functional collaboration.

Qualifications

• Registered health professional with a current Annual Practicing Certificate.
• Minimum of five years’ relevant clinical experience.
• Knowledge of ACC Nursing Services preferred.
• Proven leadership, coaching and service improvement capability.
• Strong verbal and written communication skills, with confidence using digital systems.
• Full or restricted New Zealand driver license.

Responsibilities

• Providing day-to-day clinical triage and making timely, clinically appropriate decisions at entry to service.
• Coordinating clinical administration from referral through to discharge, including documentation, prescriptions and consumable requirements.
• Supporting allocation and rescheduling decisions where clinical oversight or input is required.
• Contributing to contractual, compliance and reporting requirements, including quality activities and service improvement.
• Building effective relationships with funders, referrers, internal teams, tāngata and whānau.
