Access Community Health is seeking a proactive Care Coordinator to join our regional team in Christchurch. You will be the vital link between clients and Support Workers, coordinating rostering and ensuring high-quality, timely care for people living at home.

The 6-month fixed-term role focuses on matching workers to clients, clear communication, and maintaining accurate records while upholding privacy standards. Join a team devoted to helping people live safely at home.

Qualifications

• Clear communication and client-focused approach required.
• Ability to coordinate complex rosters and schedules.
• Experience in health, care, or aged-care settings is preferred.
• Demonstrated privacy and data-handling practices.

Responsabilités

• Coordinate and adjust daily client rosters to ensure safe, timely care.
• Match Support Workers to clients based on skills and availability.
• Communicate clearly with clients, whānau, and Support Workers.
• Respond to queries professionally and manage low-level concerns.
• Maintain accurate records and follow privacy and organisational procedures.
• Escalate risks or safety issues to the Team Leader when required.
