Department: Social Services / Patient Services / Care Coordination
Employment Type: Full-time / On-site
Experience Range: 3 to 10 Years (with experience in hospital discharge planning, care coordination, or social services)
Salary Range (INR): ₹3,50,000 – ₹8,00,000 per annum (depending on experience and hospital category)
Education: Bachelor’s / Master’s Degree in Nursing / Social Work (MSW) / Healthcare Administration / Public Health (Mandatory)
Certification in Case Management / Patient Navigation / Palliative Care (Preferred)
Familiarity with clinical workflows, insurance processes, and social support systems is essential
Responsibilities
- Coordinate comprehensive care plans for patients from admission to discharge and post-hospitalization.
- Conduct assessments of patient and family needs, including social, emotional, financial, and clinical aspects.
- Work with doctors, nurses, social workers, and families to ensure continuity of care.
- Facilitate discharge planning, home care setup, rehabilitation, or transfer to other healthcare facilities.
- Liaise with insurance providers, NGOs, and government agencies to enable patient support.
- Educate patients and caregivers on treatment regimens, follow-up plans, and community services.
- Monitor patient progress, compliance with care plans, and intervene to resolve barriers to care.
- Maintain accurate case records, documentation, and reports for internal and external audits.
- Support transitions of care, especially for high-risk or chronic illness patients.
- Ensure patient advocacy, ethical support, and adherence to hospital policies and regulatory norms.
- Case Planning & Coordination; Clinical Documentation & Care Mapping; Communication with Patients & Families; Knowledge of Government & Social Schemes; Multidisciplinary Team Collaboration; Problem Solving & Decision Making.
Qualifications
