Job summary

Praxis Care is seeking a Support Worker in Cork, ROI, to assist individuals in achieving independence and personal care goals within person-centred plans. The role supports team-based service delivery and adherence to care policies.

Responsibilities include implementing individual support plans, coordinating with team leaders, and maintaining accurate records. A flexible schedule, including full-time, part-time, and relief shifts, is required; sleep-ins may apply with extra payment.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Experience in health and social care (desirable).

Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans, including assistance with managing domestic or social skills and personal care.
• Work closely with Team Leaders and managers to prepare and implement individual and group support programmes.
• Collaborate with other Support Workers and Team Leaders to promote a safe working and living environment, resolving situations from conflict or challenging behaviours.
• Ensure that medication is stored and administered in accordance with Praxis Care’s Medication Policy where necessary.
• Familiarize yourself with administrative requirements and logging of information per service guidelines.
• Attend all related training and stay aware of policies including Health & Safety.
