Job summary

Praxis Care is seeking Support Workers to empower individuals in independent living and personal care within a respectful and dignified service. The role offers various patterns, including full-time, part-time, and relief, with salaries based on 39 hours FTE and pro-rated accordingly.

Your duties include implementing person-centred plans, coordinating with Team Leaders, ensuring safety, and adhering to health and safety policies. A driving licence and flexibility are valued.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Desirable: Experience working within the health and social care field.

Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans.
• Work closely with Team Leaders and managers to prepare and implement individual and group support programmes.
• Collaborate with other Support Workers and Team Leaders to promote a safe working and living environment, resolving situations arising from conflict or challenging behaviours.
• Ensure that medication is stored and administered in accordance with Praxis Care’s Medication Policy where necessary.
• Familiarize yourself with all administrative requirements and record information according to reporting guidelines.
• Be aware of all policies and participate in related training.
