Job summary

IREWIL CARE LIMITED is seeking a Care Co-ordinator to support the Care Manager in monitoring and developing services for Service Users in line with the company's Quality Assurance Policy.

The role involves liaising with Care Managers, HSE and other health professionals, maintaining client activity on the IT system and creating staff rotas across multiple care packages to ensure continuity of care. Hours are Mon-Fri 8:30am–5:00pm or 9:00am–5:30pm, with weekend work as needed.

Qualifications

• Advanced computer skills including proficient use of MS Office.
• Previous administrative experience within health or social care.
• Previous Care Co-ordinator experience.
• Excellent organizational and planning skills.
• Teamwork and relationship management skills.
• Strong communication skills including telephone

Responsibilities

• Maintain all client activity on IT system.
• Creation and maintenance of accurate staff rotas across multiple care packages to ensure continuity of care.
• Use excellent time management and organisation, along with forward planning to meet tight deadlines.
• Be flexible and capable of assisting colleagues in the department as and when necessary.
• Prepare handover materials at the end of shift.
• Represent the Company in a professional manner on telephone, in person or in writing.
• Maintain confidentiality and adhere to the Company's Confidentiality Policy.
• Report to the Care Manager, Assistant Manager and Director of Care any issues regarding safeguarding of clients.
• Ensure continuous improvement of service delivery.
