An exciting opportunity for a senior mental health practitioner within the living well service in Tameside & Glossop. This post involves working with Pennine Care NHS Foundation colleagues & partners in the Living Well service to identify & support patients struggling with their mental health in the community.
Senior Mental Health Practitioners conduct biopsychosocial assessments & initial conversations with individuals presenting mental health difficulties, social care needs & risk issues at a primary care level. The goal is to promote recovery & help individuals maintain their lives within their community environment.
Key Responsibilities:
Assess risk, develop risk management plans with service users as part of the Duty function. Promote self-management, facilitate engagement with other parts of LWT, and provide advice to colleagues where necessary.
Offer brief interventions (up to 12 weeks), including coping strategies, problem-solving, motivational & behavioral activation, anxiety management, risk management, suicide prevention, medication support, social & interagency support, psychological support, biopsychosocial education, resource signposting, and review of treatment plans for complex cases or those not meeting secondary care criteria.
Requirements:
Applicants must possess RMN / RNMH / Social Worker / OT or equivalent qualifications. Completion of Preparation for Mentorship / Practice Teacher / is desirable, along with significant post-registration experience.

The team provides comprehensive biopsychosocial assessments for individuals who may need secondary care and offers short-term support for identified mental health issues. As a Senior Mental Health Practitioner, you will be part of a multidisciplinary team managing patients with complex needs, conducting mental health & risk assessments, and providing short-term interventions to prevent escalation & crisis.
Skills required include mental health & risk assessment, evidence-based intervention delivery, excellent communication, relationship building, and organizational skills.
