WithinEndocrinology/Bone Health we are looking for a Band 6 Specialist Nurse who isenthusiastic and highly motived to work as part of a dynamic team.
TheBone Health multidisciplinary service, aims to identify, investigate andinitiate treatment. We work with local healthcare partners to deliver amulti-agency holistic service for people over the age of 50 within the localarea who have suffered a fragility fracture, with the aim of reducing theirrisk of subsequent fractures.
Thesuccessful applicants will be responsible for identifying these patients, andin conjunction with other members of the healthcare team, co-ordinate,provide advice, support and education to the patient in relation to on-goingtreatment and prevention of secondary fractures.
Thespecialist practitioner will contribute to the care of the patient group byleading on defined aspects of care and communicate with patients in ways thatempower them to make informed choices about their health and healthcare.
Beeducated to nursing degree level
Havea minimum of 2 years post registration nursing experience
Ifyou have an interest in endocrinology and bone health, have excellentcommunication and organisational skills, are dynamic and confident with theability to work well within a small team, but also work autonomously withinthe area of endocrinology and bone health, then you may be who we are lookingfor.

The post holder is responsible for themanagement of a defined caseload of patients. The post holder will be responsible foridentifying, assessing and recommending / initiating treatment for patients whohave suffered a fragility fracture in patients over the age of 50; with the aimof reducing their risk of subsequent or secondary fractures. The post holderwill also contribute to the initial development of the Fracture Liaison Serviceand the on-going subsequent improvement of the Service in LTHT and wider, usingresource costeffectively.
Thepost holder will be expected to request DXA scans following appropriate trainingas part of their role and will be expected to complete the nurse prescribingcourse whilst in the post.
