Overview

Successful applicants will work as part of a Multidisciplinary Team. You will hold a designated caseload and work generically as a care coordinator alongside your Nursing functions. The Trust supports regular Nursing forums, ongoing learning, and opportunities for development, with supervision, annual appraisals, and a robust in-service training/educational programme.

Responsibilities
  • Be responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and the carer.
  • Maintain a high standard of individualised client care, recognising factors and events affecting the mental, emotional and physical well-being of clients/carers, and provide advice and support as required.
  • Act as a care coordinator in assessing clients\' needs and strengths through a collaborative approach, ensuring care plans are implemented and regularly reviewed/updated.
  • Develop and maintain a high standard of practice based on assessment, using a range of psychosocial approaches within a health and psychosocial framework.
  • Communicate/liaise effectively within relevant agencies to ensure an integrated programme of care is provided.
  • Implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies.
  • Supervise and monitor the therapeutic effects and side effects of prescribed medication, providing clear advice and information in accordance with Trust Policy and NMC Standards for administration of drugs.
  • Organise and participate in admission, discharge and transfer of clients in accordance with Trust Policies.
  • Provide clients and carers with verbal and written up-to-date information on services, how to express their views, the complaints procedure, and what they can expect from the service.
  • Attend weekly clinical meetings to provide the team with information on caseloads, referrals, and feedback on new assessments.
  • Develop and maintain effective working relationships with in-patient facilities regarding admission and discharge of clients.
  • Facilitate carers\' assessments, provide appropriate care plans to meet identified needs, and monitor efficacy.
  • Demonstrate knowledge and application of key legislation and policy such as the Mental Health Act, Care Programme Approach, Capacity Act, and Care Act.
  • Participate in the running of the clozapine and depot clinics.

Post Details

We are seeking an experienced and enthusiastic Band 6 Community Psychiatric Nurse to work within community services in the borough of Merton for South West London and St George\'s NHS Trust, within the Community Recovery Support Team. The post offers a chance to work as an integrated member of a dedicated community team, providing time-limited interventions to support personal recovery and effective management of symptoms in partnership with individuals to develop recovery action plans. Applicants should have post-qualification experience in a mental health setting, ideally in a community mental health team. You should be flexible, energetic, have a positive attitude, and be able to work autonomously. Excellent communication and interpersonal skills and a collaborative, problem-solving approach are essential, with a commitment to MDT and partnership working (e.g., Recovery College and voluntary, statutory, and independent agencies).

  • Experience working within a mental health setting post-qualification; ideally in a community mental health team.
