A community health organization in London is seeking a compassionate and motivated Health & Wellbeing Coach to join the Vital 5 service in Greenwich. You will carry out health assessments and provide one-on-one coaching to residents, helping them achieve meaningful health goals. Responsibilities include maintaining records, supporting residents, and providing ongoing assistance over several sessions. The role requires teamwork and empowerment of residents to enhance their wellbeing and access necessary support.

We are looking for compassionate and motivated people to join the Vital 5 Health and Wellbeing Coaching service within Valentine PCN, as a Health & Wellbeing Coach, in Greenwich, supporting residents living in the East and Central East neighbourhoods (Thamesmead, Woolwich & Plumstead). This role will include carrying out the Vital 5 check with residents & then offering personalised, one-to-one coaching to these residents, helping them identify meaningful goals & overcome barriers to better health & wellbeing.
Initial priority: residents with high blood pressure.
Compassionate, motivated, and with a desire to support residents in achieving health and wellbeing goals.
Base of this role is within Valentine PCN and will travel across the PCN to various locations as required.
