Job description

Working autonomously, the PracticeNurse will be responsible for a number of clinical areas such as public healthand screening programmes, including infection prevention and control, chronicdisease management, immunisations, health promotion and prevention, wellman/woman clinics and cervical screening, Furthermore, they embed populationhealth management approaches within general practice, targeting healthinequalities and improving access.

The post-holder will be an integralpart of the general practice team, as well as supporting the wider practicenurse network and multi-disciplinary team. They are to participate in formaltraining events and to promote best practice in their area of expertise as wellas supporting the practice management team in the reviewing and delivery ofclinical policy and procedure.

Thisorganisation must ensure that the post-holder has access to appropriateclinical supervision and an appropriate named individual in the organisation toprovide general advice and support on a day-to-day basis. Further informationon clinical supervision can be sought within the organisations ClinicalSupervision Policy.

Main duties of the job
  • Support theInfection Prevention Control (IPC) lead and work to the IPC Policy

About us

We are based in modern premises, purpose built approximately 13 years ago, serving a growing list size of approximately 10,300 patients, covering the town and semi-rural environ of Newton Abbot, and are rated Good by the CQC.

Albany Surgery is a member of the Newton West Primary Care Network, a thriving PCN serving 35,000 patients. We work closely with Kingskerswell and Ipplepen and Bovey Tracey and Chudleigh practices, the other practices within our PCN. This collaborative approach is supported by a PCN support team, with a lead manager, social prescribers and health and wellbeing coaches working across the member practices.

As a practice we are committed to staying at the fore front of changes within General Practice, as such we recognise that the support, growth and training of all levels of our staff, both clinical and administrative, is an essential part of the current and future NHS. Education and learning are our core passions and we have 3 GP trainers hosting a number of GP registrars, we support medical student training placements of all years, and mentor health care professionals undertaking Prescribing qualifications and Advanced Practice postgraduate training. Job responsibilities

The following are the core responsibilitiesof the Practice Nurse in delivering health services:

  • Support theInfection Prevention Control (IPC) lead and work to the IPC Policy
  • Consult with patients and their families to undertakeassessments of patient need
  • Support in the long-term condition management within theorganisation, including being involved with quality improvement and assuranceinitiatives
  • Develop, implement and embed an effective care managementprogramme for the frail and housebound patients registered with theorganisation
  • Support public health and screening programmes, includingimmunisations, vaccinations, and cervical screening and also provideopportunistic health promotion advice
  • Embed population health management approaches withingeneral practice, targeting health inequalities and improving access
  • As required, provideroutine nursing care to patients in accordance with clinical based evidence,NICE and the NSF
  • Support new-to-practice nurses and other healthcareprofessionals within the organisation
  • Teach and advise patients and their families on how tomanage their condition
  • Participate in clinical audits and research projects andimplements changes as required, including supporting with the development,and updating of practice protocols/guidelines and procedures locally
  • Work within national and local protocols where theseexist
  • Recognise the boundaries of their practice and know whenand to whom patients should be referred
  • Support in the delivery of enhanced services and otherservice requirements
  • Be aware of duties and responsibilities regarding currentlegislation and adhere to practice policies and procedures on SafeguardingAdults and Safeguarding Children
  • Participate in the management of patient complaints whenrequested to do so and participate in the identification of any necessarylearning brought about through incidents and near-miss events
  • Undertake all mandatory training and induction programmes
  • Contribute to and embrace the spectrum of clinicalgovernance
  • Support the practice team with the reviewing andimplementation of practice policies and protocols, ensuring conformance toextant legislation
  • Attend a formal appraisal with their manager at leastevery 12 months. Once a performance/training objective has been set, progresswill be reviewed on a regular basis so that new objectives can be agreed
  • Lead on public health campaigns (e.g., flu clinics)
  • Provide wound care (ulcer/Doppler, etc.) to patients
  • Maintain accurate clinical records in conjunction withextant legislation ensuring that SNOMED CT codes are used effectively
  • Contribute to practice targets (QOF, etc.), complyingwith relevant guidance
  • Maintain chronic disease registers and implement aneffective call/recall system
  • Maintain a clean, tidy, effective working area at alltimes
  • Chaperone patients where necessary.
  • Support the team in dealing with clinical emergencies
  • Liaise with external services/agencies to ensure thepatient is supported appropriately (vulnerable patients, etc.)

Theremay be, on occasion, a requirement to carry out other tasks, this will bedependent upon factors such as workload and staffing levels

In addition to the primary responsibilities,the Practice Nurse has the following wider responsibilities:

  • Develop, review andimplement nursing protocols in conjunction with the partners
  • Participate in localinitiatives to enhance service delivery and patient care
  • Support andparticipate in shared learning in order to improve patient care
  • Coordinate thenursing team rota, ensuring sufficient staff are available to meet patientneeds
  • Support with theplanning and implementation of changes within the nursing team, enhancing theprovision of services to patients

Experience
  • Broad knowledge of general practice and experience of working as a Practice or Community Nurse

  • Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and womans health (cervical cytology, contraception etc.)

  • Ability to promote best practice regarding nursing matters
  • Ability to work within own scope of practice and understand when to refer to GPs
  • Good clinical system IT knowledge and an ability to record accurate clinical notes
