  • To increase the standards of palliative and end of life care across the Trust through the provision of direct patient care, educating other health care professionals and developing services.
  • The successful applicant will work as part of a well-established dynamic multi disciplinary team who provide specialist advice and support to patients, their relatives and all members of the health care teams with the hospital. The service is offered to patients with both malignant and non-malignant disease. You will be responsible for managing a caseload of patients with specialist needs. The team provides a 7 day specialist palliative care visiting service within the hospital.
  • Frimley Health NHS Foundation Trust provides NHS hospital services for around 900,000 people across Berkshire, Hampshire, Surrey and south Buckinghamshire.
About the Trust

As well as delivering excellent general hospital services to local people, we provide specialist heart attack, vascular, stroke, spinal, cystic fibrosis and plastic surgery services across a much wider area. We have three main hospitals - Frimley Park in Frimley near Camberley, Heatherwood in Ascot and Wexham Park near Slough. Our three core values, and the behaviors that support them, guide everything we do and set out what we expect of our staff in the way they treat patients, visitors, service users and each other, Committed to Excellence, Working Together and Facing the Future. We are also proud to host the Defence Medical Group South East at Frimley Park with military surgical, medical, and nursing personnel working alongside the hospital's NHS staff providing care to patients in all specialties.

Practice and Team Collaboration

  • Demonstrate patient centred approach to the assessment planning
  • Implementation and evaluation of care for palliative care patients through the use of a holistic assessment approach
  • Provide excellent symptom management to patients and refer complex cases to the appropriate specialist practitioner
  • Work collaboratively with the Palliative Medical Consultants and other members of the palliative care team to promote a multi-disciplinary approach to care across the Trust
