Job summary

The Hillingdon Hospitals NHS Foundation Trust is seeking a Patient Pathway Coordinator to lead the tracking of outpatient RTT pathways. You will work closely with clinical and managerial staff to provide high-quality administrative support, ensuring patients receive timely treatment and NHS targets are met.

You will monitor care pathways, manage referrals, maintain accurate records and support audits, reporting any delays or breaches to management.

Qualifications

• Experience coordinating patient pathways or similar care processes.
• Strong administrative skills with attention to detail.
• Ability to work with clinicians, managers and GP practices.
• Proven ability to manage data, maintain records and support audits.

Responsibilities

• Monitor and coordinate patients through their care pathway using tracking lists.
• Manage referrals and pathway administration per Trust and national requirements.
• Collaborate with clinicians, managers and GP practices to resolve delays.
• Maintain accurate patient records and secure health records.
• Coordinate information for clinical teams and MDT meetings; follow up actions.
• Collect and analyse pathway data; support audits and report issues.
