Job summary

Gloucestershire Hospitals NHS Foundation Trust is seeking a motivated Vascular Pathway Tracker to support patients through the vascular treatment journey. Working with the Vascular Pathway Navigator, you will help ensure pathways are managed efficiently and in line with NHS standards.

The role focuses on monitoring pathways, maintaining accurate records on Trust systems, supporting MDT meetings, and escalating delays or issues to protect timely patient care.

Qualifications

• Educated to GCSE level with Grade C in English and Maths.
• Experience with Microsoft Office applications (Outlook, Word, Excel, PowerPoint).
• Clear communicator with excellent writing, data entry and telephone skills.
• Ability to work under pressure in a busy environment and manage priorities.

Responsibilities

• Monitor patient pathways and maintain accurate records on Trust systems.
• Attend and support weekly MDT meetings and record outcomes.
• Escalate delays or issues affecting patient care to the Vascular Pathway Navigator.
• Coordinate with clinical, administrative and operational teams to progress pathways.
