Phoenix Futures in Chelmsford, Essex, is seeking a Practitioner to support Traveller communities with a multi-agency approach. You will work in a small team, holding a caseload and delivering intensive, coordinated support to access a wide range of services and sustain positive change.

The role covers activities such as outreach, advocacy, signposting, and referrals, forging links with local partners to form a robust network of support.

Formación

• Experience of working in health and social care or similar field.
• Ability to work with partner agencies and network for the client.
• Ability to manage a caseload with strong organisational skills.
• Proficiency in Microsoft Outlook and Word; familiarity with databases.
• Excellent written and verbal communication skills.
• Determination and willingness to learn.
• Passionate and enthusiastic about helping people.

Responsabilidades

• Provide outreach, advocacy, signposting and onward referrals.
• Accompany clients to appointments and coordinate with partners.
• Develop a multi-disciplinary network of support across Essex.
• Hold a caseload and deliver services in an intensive, multi-agency way.
